Lower Blepharoplasty (Lower Eyelid Surgery)

Lower blepharoplasty is a surgical procedure to improve visible bags, loose skin, and irregular contours under the eyes. Depending on the patient’s anatomy, surgery may include repositioning or reducing selected fat, refining excess skin, and supporting the lower eyelid.

The aim is not to remove every line or change the natural shape of the eyes. Treatment focuses on improving the relationship between the lower eyelid and upper cheek while preserving expression, facial identity, and normal eyelid function.

Why Under-Eye Fullness Can Become More Defined

Why Under-Eye Fullness Can Become More Defined

The tissues that support the lower eyelid change with age. Fat that normally sits behind the eyelid can move forward and create noticeable bulges, while thinner skin can make shadows, creases, and uneven contours more visible.

Under-eye bags can also be genetic and appear in younger patients despite good rest. The eyelid, cheek, and surrounding facial structure are assessed together to understand which anatomical changes are driving the appearance.

Features That May Be Suitable for Surgical Correction

If changes under the eyes affect the smoothness and harmony of the area, lower blepharoplasty may be considered. Common concerns include:

  • Obvious or resistant swelling under the eyes
  • Localized protrusion of lower-eyelid fat
  • Loose or wrinkled skin under the eyes
  • A hollow visible between the eye and cheek
  • Asymmetrical contour between the right and left sides
  • A tired appearance and loss of lower-eye definition

Each patient requires a different combination of correction. Some need fat repositioning, others conservative fat reduction, limited skin adjustment, or added lower-eyelid support.

Studying the Relationship Between the Eyelid and Cheek

Visible puffiness is only one part of planning lower eyelid surgery. The surgical approach is influenced by skin quality, fat position, lower-lid tone, eye shape, cheek projection, and facial symmetry.

During consultation in Riyadh, Dr. Mishal Al Shammari also reviews any prior eye procedures, dry-eye symptoms, and medical history. This helps clarify whether the concern comes from excess fat, volume loss, loose skin, or a combination of factors.

Redistributing Volume to Improve the Under-Eye Contour

Surgery may be performed through a small incision inside the lower eyelid or through a fine incision near the lower eyelashes. The chosen route depends on whether treatment is aimed at fat alone or whether skin adjustment is also required.

Excess fullness may be selectively reduced or the fat may be repositioned to soften a hollow transition. Tissue must be preserved carefully, because removing too much volume can create an over-hollowed or prematurely aged look.

Factors That Guide the Choice of Surgical Access

The incision plan is shaped by the location and amount of lower-eyelid fat, the degree of loose or wrinkled skin, the strength and position of the lower eyelid, and the depth of the groove beneath the eyes.

An internal incision may be suitable when the main issue is fat and no skin removal is needed. When excess skin also requires correction, an external incision may be placed beneath the lash line, and any visible scar usually softens as healing progresses.

Recovery While the Lower Eyelid Regains Its Natural Position

Swelling, bruising, tightness, and temporary dryness, watering, or sensitivity may occur during early healing. Patients may be advised to use cold compresses, keep the head elevated, and follow prescribed eye-care instructions.

Contact lenses, heavy exercise, and long computer sessions may need to be limited for a period. Follow-up visits monitor healing, eyelid position, symmetry, and the developing contour as swelling gradually improves.

Smoother Continuity From the Eyes Toward the Cheeks

Lower blepharoplasty aims to reduce the prominence of the under-eye contour while preserving the patient’s natural expression. Results should be assessed by the harmony between the eyelids, cheeks, and surrounding facial features rather than by the total absence of wrinkles.

The final appearance develops gradually as swelling subsides and tissues settle. Ageing continues to affect skin and facial volume over time, but conservative planning, realistic expectations, and appropriate aftercare help support a natural and balanced result.
